A.  HOMEWORK/OUT OF CLASS ASSIGNMENTS:

I.  OFF AND RUNNING ON DAY 1!
Assignments Day 1:  ALL INFORMATION BELOW IS DUE DAY 2 - PLEASE BRING ALL INFORMATION TO CLASS
1.  PRINT OUT the following information (from sources listed below on this website):
a) ..PRINT OUT ALL "FIELD Portfolio" Information listed below (under PORTFOLIOS) and place in binder
b) ..PRINT OUT the Syllabus and place in FRONT of binder (Syllabus will be discussed Day 2 or 3)
c) ..PRINT OUT and complete this form titled:  Student Data Sheet
2.  Purchase ONE binder:  1 1/2 INCHES with a translucent cover (meaning you can put a piece of paper in the front of binder)
3.  Put a cover page in translucent part of EACH  binder and include the following TYPED information:
BINDER COVER:

-YOUR NAME                                                                             
-FIELD EXPERIENCE BINDER and PROFESSIONAL DEVELOPMENT BINDER                                                   
-T/R and TIME OF CLASS (8:00 or 9:25)

-Photo/Picture Optional
                    4.   On the spine of the binder, please make sure YOUR NAME is INSERTED INSIDE (not written on outside)
5.   Purchase TABS for binder and place in binder (~8 tabs is best).  I will discuss what to do with the Tabs at a later date.
6.   Bring binder to class on Days 2 and 3 for full participation credit (I WILL CHECK)

TB/MANTOUX (This takes 3 days!  Shot Day 1,
Wait For It, Day 3 get results):
A COPY OF THE RESULTS ARE DUE to Dr. DiCorcia BY FEBRUARY 10, 2009 : If you do not provide me with a copy of your Mantoux/TB results by FEBRUARY 10th, you will be written upas a Dispositional Student and will have to meet with the HES DEPARTMENT Student Issues Committee!
FINAL RESULTS DUE:  FEBRUARY 10, 2009


FALL TB/MANTOUX (This takes 3 days!  Shot Day 1, Wait For It, Day 3 get results):
